211 Connects partners with the Montgomery Area Community Wellness Coalition to be the single point of entry for access to health and community programs for the uninsured in the River Region. This partnership assists in enhancing chronic disease prevention efforts focused on reducing the burden of diabetes, high blood pressure, obesity, and asthma and addresses three related risk factors - physical inactivity, poor nutrition, and tobacco use.
The Montgomery Area Wellness Coalition is a community-managed, nonprofit organization of health and human services providers who share information and coordinate services to increase quality, efficiency and effectiveness of services within the River Region.
These public, private and faith-based providers came together in 1999 in Montgomery, Alabama to seek solutions to reduce economic and health burdens due to lack of health insurance. They incorporated in 2002 as the Montgomery Area Community Wellness Coalition for the following purposes:

Identify and quantify the needs of the medically uninsured and underinsured population in the Montgomery, Alabama area

Strengthen the health services infrastructure by coordinating the efforts of various agencies providing services to this medically uninsured and underinsured population

Increase access to appropriate services for improving health and wellness.
Wellness Case Advocates can help you find a doctor and link you with needed services to manage your diabetes, asthma, heart condition, hypertension, obesity, and/or mental illness. Each person has a wellness case manager and/or wellness advocate who can help you set health and wellness goals. All information is private.
Who qualifies for Wellness Case Management? Anyone who does not have health insurance and who has one or more of the following conditions can receive wellness case management services:
- Asthma, diabetes, Heart Disease, Obesity, Hypertension, and/or mental illness - Frequent use of the hospital emergency room
